What Is Support Coordination?
A service funded by the NDIS, which will assist you to build the skills you need to understand and use your plan. Support Coordinators work with you to ensure a mix of supports are used to increase your capacity to live more independently, manage your plan and goals and be included in your community.

If you think we are the right fit, there are a few ways you can apply to the NDIS for us to be your Support Coordinators:
If you are Plan-Managed, your planner can help you make a request for service.
If you are Self-Managed, you can lodge a request for service by downloading the NDIS Access Request Form and emailing it to enquiries@ndis.gov.au.
Or, if you need more help, you can apply directly with us, and our team will assist.
Upon receiving the service request and being added to your list of providers, we will arrange an initial planning meeting in person, online, or via phone.
We are keen to learn more about you and will work with your nominated family members, friends, and service providers to hear about your plan goals, current services, and hopes for the future.
